2. Using your invoice, indicate the TOTAL AMOUNT DUE at the top, from your invoice.

3. Under CUSTOMER REFERENCE ID, indicate the PATIENT’S FULL NAME or PATIENT ID.

4. Fill out your payment information to complete the transaction. You will be able to complete your payment and receive a receipt via email. IMPORTANT: BE SURE TO INDICATE PATIENT’S FULL NAME OR ID UNDER CUSTOMER REFERENCE ID. Thank you!
